Requested payout from 5D earlier today, they emailed me over a list of tasks I have to complete first. Is this pretty normal in your experience?
Please refer to the attachment and you will find the authorization
form (s) you need to fill up (handwriting) in order to process your
request.
Along with the authorization form (s) make sure to include a copy of
your ID (driver’s license) and copies of your card ending in 1242
(front and back side). Make sure the cards are signed. Also for
verification purposes please send a copy of an utility bill, cell
phone bill or insurance bill matching the address and the name on the
account.
You can send this information back to the email from where you
received this one (certification@5dimes.com) or you can send it via
fax to 1-800-559-7417.
