that&#x27;s weird,  they emailed me this :Please be informed that you can also access or get this form from the Bookmaker site by clicking the Cashier then Withdrawal Option.After completing all the documents needed,  an active Plastic MasterCard Debit Card will be sent to you and  will be funded on your next payout.  First, you must receive the Debit Card before we credit any payout to your account.

What are the fees?  Usually the sites will charge you a large fee to transfer onto the card.
