Started gambling.
Took many thousands of dollars in losses to learn how to do it. When I actually became good at it, it took even more losing to realize my compulsive tendencies would never allow me to maintain a profit.
I gave back all my money I would have saved for the past 8 years or so. Well most people mess up big time with at least 1 decision in life, so I chalk it up to life experience.
If you don't count gambling, it was loaning $10,000+ to a good friend of mine that's a deadbeat when it comes to money. Also, cosigning an automobile loan for someone with no sense of responsibility. Despite being dead broke for the majority of the past decade, I've never missed a payment on anything.
