Fields is 23/24 with 4 tds. His only incomplete pass was on a Hail Mary to end the half.

Every one of his passes have been out of shotgun. 

How has this taken 3 weeks to figure out by the coaches?

Ofcourse, take it with a grain of salt as the Broncos D gave up 70 last week.

Sean Payton could be one of the worst coaching hires in quite some time. The Broncos gave up draft picks to the Saints to get him.

 The entire coaching staff has to go, the trainer, the water boy, everyone. I thought they did a much better job last year, albeit in losing fashion. 

This year they are not buttoned up, very poor decisions with terrible clock management. 

Before Fields sack fumble, he threw every pass from shotgun. Clearly you can't have this kid turn his back to a defense. On the fumble they ran play action from under center. Denver called a CB blitz. Guess what happened when Fields turned around?

There are some coaches and coordinators in this league who just can't get out of their own way.

 You clearly know better, because to blow a 28-7 lead, against a team like Denver, a lot of things have to go wrong, and they sure did today.

I'm really not that familiar, with decisions regarding coaches and all that, but to do a complete overhaul, as you're suggesting, a breaking point has to be reached, I have no idea if they are there...

 It's easy for me to tell you what I see. It's easy for me to say what a team/coach should do.

Guessing what will or will not happen on the field pre game(aka betting) is a whole other story.
