Oahu (Hawaii) has 900,000 residents with 597 square miles. Again, much easier to control islands with small populations and less travelers.
Obviously the virus is not going to spread nearly as much which lowers the death count.
Try to keep comparisons close, otherwise you're stretching.
The US also has the largest obesity % relative to population.
Obesity is also a leading cause of complications from covid.
